Job summary

VITAS Healthcare is seeking a Registered Nurse to coordinate comprehensive palliative care. The role involves assessing physical, psychosocial, and spiritual needs and working with an interdisciplinary team to implement the plan of care.

Ideal candidates have 2+ years in acute care, case management in community/hospice settings preferred, and hold current licensure with CPR/BLS/ACLS; CHPN optional. Competitive hourly pay and patient-centered care are emphasized.

Qualifications

• Minimum 2 years of experience in an acute care setting.
• 1-2 years Case Management in a community setting preferred (home health, rehab, hospice, etc.).
• Exceptional communication and customer service skills.
• Working knowledge of Outlook, Excel and Word.

Responsibilities

• Provide expert coordination of care to patients and families.
• Complete initial and ongoing comprehensive assessments of patient and family.
• Refer to specialists and support adherence to treatment plans.
• Assist patients in accessing services and resources.
• Educate patients and families about disease processes and care options.
• Provide emotional support and counseling to patients and families.
• Support discharge planning and transitions of care.
• Protect patient rights and confidentiality in all interactions.
• Participate in on-call rotations as needed.
