St. Luke's Health Network, Inc. is seeking a qualified hospice nurse to render professional nursing services to patients and families in Monroe County.

The role emphasizes assessment, planning, implementation, and evaluation of care according to the Plan of Care and agency policies. The position includes collaboration with an interdisciplinary team, coordination with physicians and the Hospice Medical Director, and supervision of LPNs and HHAs to ensure high-quality patient outcomes.

Responsabilidades

• Determines eligibility for admission; obtains financial and statistical data; formulates initial care plan.
• Collaborates with the interdisciplinary team to assure appropriate care planning.
• Communicates with the patient’s physician and hospice Medical Director as required.
• Cooperates with the Volunteer Coordinator and supports hospice volunteers.
• Carries primary responsibility for providing comprehensive nursing care and health teaching to patients and families.
• Supervises patient care provided by LPNs and HHAs.
