• Determine eligibility for admission and formulate initial care plan.
• Collaborate with interdisciplinary team to ensure appropriate care planning.
• Communicate with patients' physicians and hospice Medical Director as required.
• Supervise and guide nursing staff and volunteers in care delivery.
• Maintain and update patient records in accordance with policy.
