Job summary

St. Luke's University Health Network is seeking an experienced Hospice nurse to provide comprehensive nursing care, health teaching and family guidance in Monroe County.

You will collaborate with an interdisciplinary team, communicate with physicians and hospice leadership, and supervise LPNs and HHAs while maintaining accurate documentation. Primary schedule is 8:00–4:30 with some on-call and weekend requirements; travel to patient homes and clinics within the service area may be involved.

Qualifications

• Graduate of an accredited school of professional nursing and active nursing license in Pennsylvania.
• BSN preferred.
• Two years’ experience as a professional nurse in nursing practice or hospice care.

Responsibilities

• Determines eligibility for admission; collects data and formulates initial care plan.
• Collaborates with interdisciplinary team to ensure appropriate care planning.
• Communicates with patient's physician and hospice Medical Director, in writing and orally as required.
• Cooperates with Volunteer Coordinator; provides counsel, guidance and training to volunteers.
• Carries primary responsibility for comprehensive nursing care, health teaching and supervision to patient and family.
• Supervises nurses and home health aides; maintains and uses patient records and documentation per policy.
