Graham Healthcare Group seeks a Registered Nurse (RN) - Hospice to provide compassionate end-of-life care in patients’ homes around Squirrel Hill, PA. The role offers 40 hours per week in 8-hour shifts with a staff employment type and a $10,000 first-year bonus opportunity.

The RN will perform assessments, manage symptoms, educate families, and document care in the EMR. Travel within the territory is required as part of patient-centric care.

Qualifications

• Active RN license in the state you work in.
• Two years of RN experience; hospice experience preferred.
• Current driver’s license; ability to drive ~20% of the day

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients’ homes.
• Perform comprehensive patient assessments and symptom management.
• Develop and implement end-of-life care plans in collaboration with an interdisciplinary team.
• Educate and support patients and families through the journey.
• Maintain thorough, compliant EMR documentation.
• Travel to patient homes within the assigned territory
