Qualifications

• Active Registered Nurse license in the state you work.
• One year of RN experience delivering patient care in home health.
• Current driver’s license and ability to travel ~20% of the day.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management and treatment plans.
• Think critically, solve problems, and ensure thorough EMR documentation.
• Travel to patient homes within assigned territory.
