Job summary

AdvisaCare Hospice is seeking a Per Diem Hospice Registered Nurse for Port Huron, MI. This role delivers compassionate, one-on-one hospice care in patients’ homes, with on-call responsibilities to ensure continuous support for patients and families in their final days.

You will assess needs, coordinate with the care team, supervise paraprofessionals, and document care plans and progress. A Michigan RN license and hospice experience are required, with competitive pay and flexible scheduling.

Qualifications

• Current Michigan license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Ability to work a full-time on-call schedule.

Responsibilities

• Provide one-on-one home-based hospice care.
• Support patients and families with dignity and heart.
• Supervise paraprofessionals delivering services.
• Participate in patient/family plan of care and IDG conferences.
• Prepare clinical/progress notes and maintain documentation.
