AdvisaCare Hospice in Muskegon, MI is seeking a Full-Time Hospice Registered Nurse to deliver compassionate, in-home care. Provide one-on-one support to patients and families with dignity and comfort, while collaborating with a multidisciplinary team to tailor plans of care.

You will conduct supervisory visits, participate in IDG meetings, and be available for on-call duties. A Michigan RN license and at least one year hospice experience are required; empathy and adaptability to changing patient

Qualificações

• Current Michigan RN license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Participation in field visits daily and weekly office meetings as indicated.
• Ability to work full-time on-call and adapt to changing patient needs.

Responsabilidades

• Provide one-on-one in-home care.
• Support patients and families with dignity and compassion.
• Spend meaningful time with patients where it matters most.
• Collaborate with the care team to tailor plans of care.
• Conduct supervisory visits and provide feedback to staff.
• Participate in interdisciplinary group meetings (IDG).
• Help develop, review and revise patient/family plans of care.
• Advocate for patients and families' needs and wishes.
