Episcopal Homes of Minnesota is seeking an experienced Registered Nurse for After-Hours Triage in Oakdale, MN. This per-diem role focuses on telephone triage, symptom assessment, and selective in-person visits to support hospice patients and families during evening and overnight hours.

You’ll work independently, with on-call shifts and comprehensive documentation, collaborating with the hospice team to provide compassionate care.

Qualifications

• Minnesota RN license required.
• Strong assessment and critical-thinking skills.
• Ability to work independently.
• Valid driver’s license and reliable vehicle.
• Hospice, home health, acute care, or triage experience preferred.

Responsibilities

• Provide after-hours telephone triage for hospice patients and families.
• Assess symptoms and determine appropriate interventions.
• Make in-person visits when needed.
• Provide comfort-focused nursing care and symptom management.
• Educate and support patients, families, and caregivers.
• Collaborate with physicians and the hospice interdisciplinary team.
• Complete timely and accurate clinical documentation.
