Qualifications

• Active RN license required.
• Hospice knowledge and experience preferred.
• Experience with after-hours triage and on-call nursing.
• Strong communication and organizational skills.
• Ability to travel to patient homes as needed.

Responsibilities

• Provide after-hours on-call nursing support for hospice patients and families.
• Respond promptly to calls about changes in condition or symptoms.
• Provide telephone triage and determine need for in-person visits.
• Conduct after-hours patient visits as clinically appropriate.
• Assess and manage pain and changes in patient condition.
• Offer education and emotional support to patients and caregivers.
• Coordinate with physicians, pharmacies, and other hospice team members.
• Document calls, visits, assessments, and interventions.
• Support patients and families during end-of-life situations.
• Ensure care aligns with hospice regulations and policies.
