Qualifications

• Graduate of an accredited nursing program.
• Current State License as an RN.
• 1 year of clinical care experience.
• Hospice or home health experience preferred.

Responsibilities

• Ensure activities align with Elara Caring’s mission.
• Assess hospice patients and families to identify needs.
• Document needs and outcomes in clinical records.
• Communicate changes to the Clinical Supervisor and team.
• Provide patient/family education and bereavement support.
