Qualifications

Responsibilities

• Provide hospice care to patients in Jackson and Washtenaw County.
• Assess physical, psychosocial, and environmental needs of patients and families.
• Communicate findings to the clinical team and physician.
• Support Interdisciplinary Plan of Care and bereavement processes.
