Job summary

Pioneer Valley Hospice & Palliative Care in Massachusetts is seeking a compassionate Hospice Registered Nurse for the Evening Shift. You will triage, coordinate, and deliver hospice care to patients and families according to the plan of care.

Qualifications

• Minimum of one to two years of experience in health care.
• Experience in hospice or home health preferred.
• Experience in acute care in an institutional setting preferred.
• Understand hospice philosophy and issues of death/dying.

Responsibilities

• Demonstrates understanding of hospice philosophy and articulate to others.
• Manages plan of care from referral through discharge per regulations and standards.
• Completes initial nursing assessment to determine hospice needs.
• Provides complete physical assessment and history of illnesses.
• Implements plan of care with patient/family to promote comfort and dignity.
• Administers medications/treatments as prescribed; observes responses and adjusts as needed.
• Coordinates with interdisciplinary team for pain and symptom management.
• Documents patient condition and outcomes; maintains accurate notes.
• Completes patient discharges and communicates per regulations.
• Documents at point of service and communicates with IDT.
• Communicates needs with medical director and attending physician as required.
• Participates in interdisciplinary team meetings.
• Maintains a safe patient environment.

Education

An associate’s degree in nursing is required.

A bachelor’s degree in nursing is preferred.
