Job summary

Pioneer Valley Hospice & Palliative Care is seeking a part-time on-call Registered Nurse to join a collaborative team in Massachusetts. The role focuses on coordinating and delivering hospice care, with duties spanning assessment, plan of care, and family support.

Qualifications

• One to two years of health care experience (minimum).
• Hospice or home health experience preferred.
• Massachusetts RN license required; CPR certification preferred.

Responsibilities

• Demonstrates hospice philosophy; articulates it to others.
• Manages plan of care from referral through discharge per regulations.
• Completes initial nursing assessment to determine hospice needs.
• Performs physical assessments and documents health history.
• Implements plan of care with patient/family to promote comfort and autonomy.
• Administers prescribed medications and observes responses.
• Collaborates with the IDT to educate patients/families on pain and symptom management.
• Documents patient status and outcomes accurately in clinical notes.
• Participates in agency Quality Improvement programs.
• Maintains safe patient environments and communicates changes to supervisors.
