Job summary

Advocate Health in Milwaukee seeks an experienced Hospice Registered Nurse to provide after-hours nursing care, including comprehensive assessments, triage, and interventions aligned with hospice goals. You will assess symptoms, educate patients and caregivers, and coordinate care with physicians and the interdisciplinary team.

This role requires an active RN license in the state, BLS, and the ability to travel to patient homes.

Qualifications

• Completion of an accredited or approved nursing program.
• Active RN license in the state of practice.
• Basic Life Support (BLS) certification.

Responsibilities

• Provide comprehensive nursing assessment, triage, and intervention for hospice patients during after-hours shifts.
• Respond to patient and caregiver calls to assess symptoms, educate, and determine appropriate interventions.
• Conduct in-person visits to manage acute symptom changes and ensure patient comfort and safety.
• Manage end-of-life symptoms including pain, dyspnea, agitation, and nausea.
• Initiate and carry out physician orders and hospice protocols within scope of practice.
• Document all patient encounters and communications accurately in the EMR.
• Collaborate with physicians and the interdisciplinary team to coordinate care.
• Adhere to hospice policies and participate in education, quality improvement, and safety initiatives.
