Job summary

Visiting Nurse Association of Wisconsin, Inc. seeks an experienced Hospice Registered Nurse to provide after-hours nursing assessment, triage, and interventions for hospice patients in the community.

This role requires an active RN license in Wisconsin, BLS certification, and at least one year of hospice or equivalent experience, with strong communication and independent clinical decision-making skills.

Qualifications

• RN license in the state of practice (Wisconsin) required.
• BLS certification required.
• Hospice/end-of-life care experience or exposure preferred.

Responsibilities

• Provide comprehensive nursing assessment, triage, and intervention for hospice patients during after-hours shifts.
• Respond to patient and caregiver calls to assess symptoms and educate in alignment with hospice goals of care.
• Conduct in-person visits to manage acute symptom changes, provide crisis support, and ensure patient comfort.
• Administer and follow physician orders and standing hospice protocols within scope of practice.
• Document all encounters accurately in the electronic medical record with clear handoffs for continuity of care.
