aah is seeking a compassionate Registered Nurse to join the 34141 WI Hospice Divisional - Nursing: SCT (Supportive Care Team) in Milwaukee North and surrounding areas. This full-time role offers a 36-hour workweek with a Friday–Sunday 08:00–20:00 shift and after-hours hospice support.

The job emphasizes comprehensive patient assessment, symptom management, and family education, with collaboration across the interdisciplinary team.

Qualifications

Completion of an accredited nursing program.
Active RN license in the state of practice.
BLS certification - American Heart Association.
Typically one year of nursing experience in any clinical setting.
Strong communication and crisis-management skills.

Responsibilities

Provide comprehensive nursing assessment and after-hours triage for hospice patients.
Respond to calls to assess symptoms and determine interventions aligned with hospice goals.
Conduct in-person visits to manage acute symptom changes and ensure patient comfort.
Initiate physician orders and hospice protocols within scope of practice.
Document encounters accurately in the EMR and coordinate care with the team.
