Job summary

Advocate Health is seeking a Hospice Registered Nurse for after-hours coverage in the Green Bay area. The role involves after-hours nursing assessments, triage, and interventions for hospice patients, including home visits as clinically indicated.

You will respond to calls, educate families, and coordinate with physicians to ensure comfort and alignment with hospice goals. The ideal candidate has an active WI RN license, BLS certification, and at least one year of hospice or related experience.

Qualifications

• Completion of an accredited or approved nursing program.
• Active Registered Nurse (RN) license in the state of practice.
• Valid drivers license.
• Basic Life Support (BLS) certification American Heart Association.
• Typically one (1) year of nursing experience in hospice or related setting.
• Experience in hospice, palliative care, oncology, ICU, ED or home health.

Responsibilities

• Provide after-hours hospice nursing assessment, triage, and interventions.
• Respond to calls from patients and caregivers; educate and determine interventions.
• Conduct in-person visits to manage acute symptom changes and ensure comfort.
• Manage end-of-life symptoms (pain, dyspnea, agitation) and safety.
• Carry out physician orders and hospice protocols within scope.
• Provide emotional support and family guidance on treatment decisions.
• Collaborate with physicians and interdisciplinary team; coordinate care changes.
• Document encounters and handoffs accurately in EMR.
• Adhere to policies, regulations, and participate in education/QA initiatives.
