Job summary

AccentCare Inc. is seeking an experienced Registered Nurse for after-hours hospice admissions in the North Dallas area. This full-time weekend position requires Friday 5p–Monday 8a coverage and on-call rotation participation.

You will conduct comprehensive nursing assessments, coordinate patient care with the interdisciplinary group, and ensure timely documentation. A valid RN license, strong communication, and teamwork are essential, with hospice/home health experience preferred.

Qualifications

• Must hold an active RN license in the applicable state.
• Associate degree in nursing preferred.
• CPR certification recommended for direct patient care.
• Hospice/home health/medical-surgical experience preferred.

Responsibilities

• Conduct comprehensive nursing assessments and develop/adjust care plans with IDG.
• Maintain timely, accurate documentation and care coordination.
• Provide education and emotional support to patients and caregivers; participate in on-call rotations and care transitions.
• Coordinate all aspects of patient care when acting as Care Manager, including scheduling and supervision of staff.
• Respond to triage calls, perform visits, and participate in IDG meetings when in on-call duties.
