Job summary

Valor Hospice in Dallas, TX is seeking an Admissions Registered Nurse to manage patient admissions and provide in-home nursing care across ages. The role requires coordinating with the interdisciplinary team, families, and referring agencies, with flexible scheduling and on-call responsibilities.

The Admissions RN will document plans of care, perform nursing assessments, and ensure infection control standards.

Qualifications

  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Current State license as a Registered Nurse; Current driver’s license.
  • Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice.
  • Good interpersonal skills.
  • Transportation: Reliable transportation and valid auto liability insurance.

Responsibilities

  • Assess hospice patients utilizing the Nursing Assessment and LCD criteria to identify the physical, psycho social, and environmental needs.
  • Develop and document the plan of care in correlation with Assessment.
  • Provides nursing care utilizing infection control measures (OSHA).
  • Work with Interdisciplinary Group/Team to ensure patient/family needs are met.
  • Communicate with attending physicians, hospice physicians, and other physicians involved in the patient’s care.
  • Timely submission of all required paperwork.
  • Demonstrates commitment and professional growth by participating in hospice sponsored in-service programs and maintaining or improving competency. Participates in the Hospice’s QAPI program.
  • Promotes the Hospice’s philosophy and administrative policies.
  • Meets productivity standards.
  • Performs on-call responsibilities and provides on-call services to patients and their families as assigned.
  • Provides effective communication to patients, their family members, team members, and other health care professionals.

Position Type: Full Time

Primary functions are to perform admissions and administer nursing care for patients of all ages in their place of residence, coordinate care with the Interdisciplinary Group/Team, patient/family, and referring agency; and assume the responsibility for coordination of care. Admission RN is a salaried position and the RN must have the ability to work a flexible schedule. Primary working hours will be Monday thru Friday 9am-6pm. Hours may vary depending on time of admission. RN will be required to hold office hours as assigned by supervisor when not assigned to admissions. RN is required to participate in on-call scheduling as assigned and other duties as assigned by supervisor.
