Job summary

3HC Home Health and Hospice in Smithfield, NC is seeking a Hospice Nurse, RN (Admissions) for a full-time position. You will coordinate hospice services, admit patients, educate families, and collaborate with physicians and the interdisciplinary group to develop and implement care plans.

The role requires NC RN licensure, strong communication skills, and the ability to supervise staff while delivering compassionate end-of-life care.

Qualifications

Registered Nurse, NC license.
Minimum of 1 year as a professional nurse in acute care preferred.
Evidence of leadership qualities.
Experience with terminally ill patients preferred.
Ability to communicate effectively with patients, families, staff, physicians and the public.
Maintain CPR certification (BCLS).
Dependable and able to work tactfully with patients and staff.
Competent to perform delegated tasks to support patient outcomes.
Must have valid Driver's License.

Responsibilities

Abides by 3HC Compliance Program and Code of Ethics.
Creates positive experiences for internal and external customers.
Provides comfort to patients and families and coordinates hospice care per plan of care.
Maintains current electronic documentation in EMR; 48-hour updates.
Supervises LPNs and paraprofessionals per CMS/Hospice guidelines.
Maintains regular communication with supervisor and IDG team.
Covers productivity and participates in on-call rotations as needed.
