AccentCare, Inc. in Galveston is seeking a dedicated Hospice After Hours Admission RN. This full-time role covers the Galveston area with a 7 on, 7 off schedule and a pay range of $87,000–$92,000 per year.

Responsibilities include comprehensive nursing assessments, care coordination with IDG, and on-call admissions. Must hold RN licensure, prefer an associate degree, and have experience in hospice or related clinical settings; willingness to participate in on-call rotations and travel within the

Qualifications

• Active RN licensure in the applicable state is required.
• Willingness to participate in on-call rotations and maintain transportation.
• Experience in hospice, home health, or related nursing is preferred.

Responsibilities

• Perform comprehensive nursing assessments with IDG collaboration.
• Coordinate care plans, ensure regulatory alignment and proper documentation.
• Communicate effectively with care teams and patients/families.
• Manage on-call admissions and care transitions.
• Supervise appropriate staff when in a Care Manager role if needed.
