Job summary

AccentCare is seeking a dedicated RN for after-hours admissions in Galveston. You will perform comprehensive nursing assessments, coordinate with an interdisciplinary group, and ensure accurate care plans with timely documentation.

Responsibilities include admissions, on-call rotations, and education for patients and families. The role may involve care coordination, scheduling, and supervision of clinical staff as needed, in a supportive hospice environment.

Qualifications

• Current licensure as a Registered Nurse is required.
• Associate degree in nursing preferred.
• CPR certification recommended.
• Willingness to participate in an on-call rotation and maintain reliable transportation and a valid driver's license.
• Proficiency in Microsoft Office and EMR systems; ability to collaborate in an interdisciplinary team.
• Physically capable of lifting patients and moving supplies as needed.

Responsibilities

• Conduct comprehensive nursing assessments and collaborate with patients, families, physicians, and the IDG to develop and evaluate care plans.
• Maintain timely documentation and ensure care plans reflect patient status.
• Provide education and emotional support to patients and caregivers and participate in admissions and on-call rotations.
• Coordinate care as a Care Manager or On-Call, including scheduling, medication management, and supervision of care staff.
