Job summary

Interim HealthCare Hospice in Great Bend, KS is seeking a compassionate Hospice Registered Nurse (RN) to provide high-quality, home-based care to patients with six months or less to live. You will work as part of an interdisciplinary team including a physician, LPN, CNA, HHA, social worker, chaplain, and volunteers to carry out the patient’s plan of care.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license in Kansas.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency, communication skills and compassion.

Responsibilities

• Provide home-based care to patients with six months or less to live, focused on quality of life.
• Work as part of an interdisciplinary team to carry out the patient’s plan of care.
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medications and treatments.
• Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies.
• Educate family on the patient’s prognosis, changes in care plan, medications and hospice goals.
• Take a holistic approach to care engaging the patient’s mind, body, spirit and family.
