Qualifikationen
- Graduate of an accredited nursing program and active RN license in Kansas.
- CPR certification required.
- Clinical proficiency, strong communication skills and compassion.
Aufgaben
- Provide home-based care to patients with illnesses, injuries and chronic diseases.
- Work with a team of physicians and oversee LPNs, CNAs and aides.
- Examine patients, do assessments, document progress and report changes.
- Educate patients on plan of care, medication, therapy, diet and exercise.
- Coach family members on plan of care and home safety.
- Coordinate discharge from services and ensure goals are met.
