Job summary

Interim HealthCare Hospice in Great Bend, KS is seeking a compassionate Hospice RN to provide home-based care and support to patients with life-limiting illness. You will work within an interdisciplinary team to implement the patient’s plan of care and ensure comfort and quality of life.

As an RN with Kansas licensure and CPR certification, you will educate families, perform assessments, monitor symptoms, and coordinate equipment and medications.

Qualifications

• Graduate of an accredited nursing program with active RN license in Kansas.
• CPR certification is required.
• Knowledge of state and federal home health regulations is preferred.
• Strong communication skills and compassion for patients and families.

Responsibilities

• Provide home-based care to patients with six months or less to live, focused on quality of life.
• Work as part of an interdisciplinary team with physicians, LPN, CNA, HHA, social worker, chaplain and volunteers.
• Perform patient assessments, check vitals, monitor breathing, manage pain, and administer prescribed medications and treatments.
• Ensure accessibility of medical equipment, oxygen, medications and supplies.
• Educate families on prognosis, care plan changes, and hospice goals.
• Take a holistic approach to engage patient, mind, body, spirit, and family.
