Job summary

Interim HealthCare seeks a PRN Hospice Registered Nurse to provide compassionate, home-based care for patients with six months or less to live. You will work with a physician, LPN, CNA and other professionals to implement the patient plan of care.

Responsibilities include patient assessments, vitals, pain management and education for families. Nursing license in Wisconsin and CPR are required, with at least 1 year of hospice/home health experience preferred.

Qualifications

Graduate of an accredited nursing program and active RN license.
Minimum of 1 year nursing experience, ideally in hospice or home healthcare.
CPR certification.
Knowledge of state and federal home health regulations.
Clinical proficiency in nursing, strong communication and compassion.

Responsibilities

Provide home-based care to patients with six months or less to live, focused on quality of life.
Work with interdisciplinary team to carry out patient plan of care.
Perform patient assessments, check vitals, monitor pain, administer medications and treatments.
Ensure accessibility of equipment, oxygen, medications and supplies.
Educate family on prognosis, changes in care plan and hospice goals.
Take a holistic approach to care involving patient, family and mind/body/spirit.
