Job summary

James River Hospice in Virginia is seeking a compassionate Hospice RN Case Manager to join our growing team. You will oversee hospice case management for patients in their homes on an intermittent basis and coordinate with the interdisciplinary team to ensure comfort and quality of life.

The ideal candidate is a Registered Nurse with Virginia licensure, at least one year of hospice experience, a valid driver's license, and reliable transportation.

Qualifications

• Must be a Registered Nurse with a current license to practice nursing in Virginia.
• At least one year of hospice experience is required.
• Graduate of an accredited nursing school.
• Must have a valid driver's license and a reliable car available for use during work hours.

Responsibilities

• The Hospice RN completes an initial patient/family assessment and develops an individualized plan of care focused on comfort and quality of life; coordinates care with the interdisciplinary team; supervises LPNs and hospice aides; provides hands-on pain and symptom management; participates in on-call rotation per policy.

Salary Range: $95k to $105k

Brief Overview of Responsibilities:

• The Hospice RN is responsible for completing an initial assessment of the patient and family to determine hospice needs and developing an individualized plan of care focused on comfort and quality of life. This role includes managing and coordinating care with the interdisciplinary team, supervising LPNs and hospice aides, providing hands‑on support for pain and symptom management, and spending meaningful time with patients and families. The RN monitors, documents, and communicates changes in condition, obtains and carries out physician orders, adjusts the plan of care as needed, participates in on‑call rotation per policy, and performs other duties as assigned.
