Job summary

James River Hospice is actively seeking a compassionate Hospice RN (On-Call) to join our Lynchburg, VA team. You will manage hospice cases intermittently at patients' homes and coordinate with the interdisciplinary team for optimal patient care.

The role requires an RN license in Virginia, with at least one year of hospice experience. We offer competitive salary, generous benefits, and flexible scheduling to support quality patient care.

Qualifications

• Must be a Registered Nurse with a current license to practice nursing in Virginia.
• At least one year of hospice experience is preferred.
• Graduate of an accredited nursing program.

Responsibilities

• Complete an initial assessment of the patient and family to determine hospice needs.
• Develops a care plan that establishes goals based on nursing diagnosis and palliative actions.
• Provides coordination among the clinical team supporting the patient and supervising LPNs and hospice aides.
• Spend quality time with the patient to assist with pain management and support.
• Modify and re-evaluate the plan of care when necessary.
• Monitor, record, and report symptoms and changes in the patient’s condition.
• Obtain and receive physician orders as required.
• Participates in on-call duties as defined by the on-call policy.
• Other duties as assigned.

James River Hospice is actively seeking a compassionate and highly committed Hospice RN (On-Call) to join our growing team in Lynchburg, VA! You will be responsible for the hospice case management of your assigned patients on an intermittent basis in their place of residence. You will also coordinate with the interdisciplinary team to ensure an optimal care experience for the patient!

Schedule: (Friday @ 5:00 PM- Monday at 8:00 AM)

Salary Range: $72,000 - $82,000

• Graduate of an accredited nursing school.
• Must have a valid driver's license and a reliable car available for use during work hours.
