Job summary
James River Hospice is seeking a compassionate Hospice RN Case Manager to manage hospice care for patients in their homes within Prince William and Fauquier counties. You will assess patients, develop care plans focused on comfort, coordinate with the interdisciplinary team, supervise LPNs and aides, and participate in on-call rotation.
Qualified candidates must hold an active Virginia RN license, have at least one year of hospice experience, graduate from an accredited nursing program, and have
Qualifications
- Must be a Registered Nurse with a Virginia license to practice nursing.
- At least one year of hospice experience is required.
- Graduate of an accredited nursing school.
- Must have a valid driver's license and a reliable car available for use during work hours.
Responsibilities
- The Hospice RN conducts initial patient and family assessments to determine hospice needs and develops an individualized plan of care focused on comfort and quality of life. This role includes coordinating with the interdisciplinary team, supervising LPNs and aides, providing hands-on pain and symptom management, documenting changes, and participating in on-call rotation.
