Job summary

James River Home Health and Hospice is seeking a Hospice Aide/CNA to support patients in their homes as part of the care team in Winchester, VA. The role requires Virginia CNA certification and at least one year of hospice or home health experience.

The position offers an hourly compensation of $20–$26, with mileage reimbursement, a company-provided tablet, and a flexible PRN schedule to meet patient needs.

Qualifications

• CNA must be certified by the Virginia Department of Health Professions.
• Minimum 1 year of CNA/Hospice/Home Health Aide experience; hospice/home health preferred.
• Graduate of an accredited nursing school.
• Valid driver's license with reliable, insured vehicle.

Responsibilities

• Perform personal care services as instructed in the Aide Plan of Care.
• Document the care provided according to agency policy.
• Report deviations from the Plan of Care to the Case Manager.
• Take vital signs as indicated on the plan of care.
• Record and report changes in patient condition to the health care professional managing care.
• Provide skin care to prevent breakdowns and report concerns to the Case Manager.
• Assist with ambulation, transfers, ROM, and other support as documented.
• Assist RN with specialized procedures under RN orders.
• Participate in on-call duties per policy.
• Other duties as assigned.

We are seeking a Hospice Aide/CNA to support our patients in their homes as part of our care team.

Territory: Winchester and surrounding areas

Compensation: $20 - $26.00 per hour

Brief Overview of Responsibilities:

• Perform personal care services (bathing, dressing, toileting, feeding, etc) as instructed in the Aide Plan of Care.
• Document the care provided according to the agency policy
• Report any deviations from the Plan of Care to the Case Manager
• Take vital signs when indicated on the Hospice Aide plan of care
• Record and report changes in the patient's physical condition, behavior or appearance to the health care professional managing the patient’s care.
• Provide proper care and observation of patient’s skin conditions to prevent skin breakdowns. Report any changes or areas of concern to the Case Manager
• Assist with ambulation, transfers, range of motion, and other support as documented on the Aide plan of care.
• Assist the Registered Nurse in performing specialized procedures as directed by the Registered Nurse under the physician's orders.
• Participates in on-call duties as defined by the on-call policy
• Other duties as assigned

Qualifications:

• Certified Nurse Aide (CNA) with the Virginia Department of Health Professions (Required)
• One year of experience as a CNA, Hospice Aide, or Home Health Aide. Home Health or Hospice experience is preferred
• At least one year of hospice experience is preferred.
• Graduate of an accredited nursing school.
• Must have a valid driver's license and a reliable, insured car available for use during work hours.
