Job summary

HopeHealth is seeking a Hospice Field Registered Nurse (RN) for its affiliate HopeHealth Hospice & Palliative Care. This per diem role focuses on urgent patient needs, requiring strong clinical assessment skills and the ability to act quickly to determine the best course of action.

The RN will perform initial and ongoing assessments, implement plans of care, educate patients and families, coordinate with the interdisciplinary team, and adjust care as needed.

Qualifications

• Graduate of an accredited nursing school; RN required; BSN preferred
• Licensed as a Registered Nurse in RI and MA
• Minimum of 1-3 years of clinical experience

Responsibilities

• Initial and ongoing assessment of patient needs across physical, functional, psychosocial, and environmental domains.
• Implements the individualized plan of care and plans for discharge/transfer as appropriate.
• Educates patients and families on disease process, self-care techniques, and care instructions.
• Initiates preventive and rehabilitative nursing procedures.
• Coordinates care with the interdisciplinary team to update the plan of care as needed.
• Informs the physician of changes in patient needs and outcomes.
• Determines scope and frequency of services based on acuity and needs.
• Coordinates care with family/caregiver to meet identified needs, including psychosocial and spiritual support.
