Job summary

Lorian Health in San Diego, CA is seeking a dedicated RN to provide compassionate hospice care in patients' homes and communities. You will perform comprehensive assessments, develop and update care plans, manage symptoms and medications, and coordinate with the interdisciplinary team to ensure high-quality outcomes.

Ideal candidates have 1+ year of Home Health or Hospice experience, hold an active RN license, and possess a valid driver's license.

Qualifications

• RN licensed in the state of practice (California preferred).
• BSN degree preferred; not required for eligibility.
• IV or ET certification recognized by the state.
• Minimum 1 year of Home Health or Hospice experience.
• Valid driver's license required.

Responsibilities

• Provide compassionate, patient-centered nursing care to hospice patients in home and community settings.
• Complete comprehensive assessments to identify patient and family needs.
• Develop, implement, and update individualized hospice and palliative care plans.
• Assess and manage pain, symptoms, medications, and treatments per plan of care.
• Serve as Case Manager when assigned and coordinate care across the hospice team.
• Educate patients and families on medications, symptom management, and end‑of‑life care.
• Communicate changes in patient condition with physicians and the interdisciplinary team.
• Maintain accurate clinical documentation and ensure continuity of care.
• Support patients and families with physical, emotional, and end-of-life care needs.
• Coordinate equipment, supplies, and services to support care quality.
• Participate in team meetings, quality improvement activities, and continuing education.

What You Will Do

• Provide compassionate, patient-centered nursing care to hospice patients and their families in the home and community setting.
• Complete comprehensive and ongoing assessments to identify patient and family needs.
• Develop, implement, and regularly update individualized hospice and palliative care plans.
• Assess and manage pain, symptoms, medications, and treatments in accordance with the patient’s plan of care.
• Serve as a Case Manager when assigned and coordinate care across the patient’s interdisciplinary hospice team.
• Educate patients and families on medications, symptom management, disease progression, and comfort-focused care.
• Communicate changes in patient condition and care needs with physicians and members of the interdisciplinary team.
• Maintain accurate, timely clinical documentation and ensure continuity of care.
• Support patients and families with their physical, emotional, and end-of-life care needs.
• Coordinate necessary equipment, supplies, and services to support quality patient care.
• Participate in interdisciplinary team meetings, quality improvement activities, and continuing education.

Required Qualifications

• Is a graduate of an accredited school of professional nursing, BSN preferred.
• Currently licensed to practice as a RN in the state of practice.
• If IV or ET certified, is currently certified and recognized by the state(s) where currently practicing as an RN.
• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
• Possess and maintains a valid driver’s license.

Pay Ranges: $53-$57 commensurate with experience
