Job summary

Lorian Health in San Diego is hiring a qualified RN for hospice care in home settings. You will provide compassionate nursing care, conduct assessments, and develop ongoing care plans to support patients and families at the end of life.

Responsibilities include pain and symptom management, coordination with an interdisciplinary team, and patient education. A valid RN license and one year of home health/hospice experience are required; BSN preferred.

Qualifications

• Graduate of an accredited nursing program. BSN preferred.
• Currently licensed to practice as an RN in the state of practice.
• If IV or ET certified, must be certified and recognized by the state(s) where practicing.
• Minimum of one year of recent experience in Home Health or Hospice setting.

• Provide compassionate, patient-centered nursing care to hospice patients and families in home/community settings.
• Conduct comprehensive assessments and develop individualized hospice/palliative care plans.
• Manage pain, symptoms, medications, and treatments per plan of care.
• Serve as Case Manager and coordinate care with the interdisciplinary team.
• Educate patients and families on medications, symptom management, and disease progression.
• Maintain accurate clinical documentation and ensure continuity of care.
• Coordinate equipment, supplies, and services to support quality patient care.

What You Will Do

• Provide compassionate, patient-centered nursing care to hospice patients and their families in the home and community setting.
• Complete comprehensive and ongoing assessments to identify patient and family needs.
• Develop, implement, and regularly update individualized hospice and palliative care plans.
• Assess and manage pain, symptoms, medications, and treatments in accordance with the patient’s plan of care.
• Serve as a Case Manager when assigned and coordinate care across the patient’s interdisciplinary hospice team.
• Educate patients and families on medications, symptom management, disease progression, and comfort-focused care.
• Communicate changes in patient condition and care needs with physicians and members of the interdisciplinary team.
• Maintain accurate, timely clinical documentation and ensure continuity of care.
• Support patients and families with their physical, emotional, and end-of-life care needs.
• Coordinate necessary equipment, supplies, and services to support quality patient care.
• Participate in interdisciplinary team meetings, quality improvement activities, and continuing education.
  

Required Qualifications

• Is a graduate of an accredited school of professional nursing, BSN preferred.
• Currently licensed to practice as a RN in the state of practice.
• If IV or ET certified, is currently certified and recognized by the state(s) where currently practicing as an RN.
• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
• Possess and maintains a valid driver’s license.
  

Range: $53-$57 per hour, commensurate with experience.
