Lorian Health is seeking a Home Health RN to provide skilled nursing care in patients' homes in the San Jose area. You will assess conditions, implement and monitor plans of care, and coordinate with physicians and families to ensure continuity of care.

The role includes administering treatments, educating patients and families, documenting thoroughly, and participating in ongoing professional development within a mission-driven team.

Formación

• RN license in state of practice required.
• BSN preferred.
• IV or ET certification as applicable.
• Minimum 1 year recent Home Health or Hospice experience.

Responsabilidades

• Provide skilled nursing care per nursing process and Plan of Care.
• Assess and monitor patient condition; re-evaluate needs.
• Administer medications, treatments, and procedures within scope.
• Communicate changes to physicians, supervisors, patients, and families.
• Educate patients and families on meds, treatment plans, disease management and safety.
• Complete accurate clinical documentation per policy.
• Promote safe, efficient care environment and supervise staff as needed.

Schedule

Monday - Friday with on call weekends rotation

As a Home Health RN, You Will

• Provide skilled, compassionate nursing care in accordance with the nursing process, physician orders, and the patient’s individualized Plan of Care (POC).
• Assess and monitor patient condition, regularly re-evaluating needs.
• Administer medications, treatments, and specialized procedures within scope of practice, training, and demonstrated competency.
• Communicate changes in patient condition and care needs with physicians, supervisors, patients, and families/caregivers to promote continuity of care.
• Educate patients and families/caregivers on medications, treatment plans, disease management, safety, and self-care to support independence.
• Complete accurate, timely, and thorough clinical documentation in accordance with Lorian policies, procedures, and regulatory requirements.
• Promote a safe, efficient care environment while supporting activities of daily living, appropriately delegating/supervising staff, and maintaining professional knowledge through ongoing education.

Required Qualifications

• Is a graduate of an accredited school of professional nursing, BSN preferred.
• Currently licensed to practice as a RN in the state of practice.
• If IV or ET certified, is currently certified and recognized by the state(s) where currently practicing as an RN.
• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.

This position is based on a pay per visit compensation model. Pay per visit ranges from $110-180 visit commensurate with experience and depending on type of visit.
