Job summary

Lorian Health in the San Diego area is seeking a compassionate RN to provide in-home hospice and supportive care. You will conduct comprehensive assessments, develop and update individualized care plans, and coordinate care with the interdisciplinary team to ensure comfort and quality of life for patients and families.

Requirements include current RN license in the state, BSN preferred, and 1 year of home health or hospice experience.

Qualifications

• Graduate of an accredited school of professional nursing; BSN preferred.
• Current RN license in the state of practice.
• IV or ET certification if applicable.
• Minimum 1 year of recent home health or hospice experience.
• Valid driver's license.

Responsibilities

• Provide compassionate, patient-centered nursing care to hospice patients and their families in the home and community setting.
• Complete comprehensive and ongoing assessments to identify patient and family needs.
• Develop, implement, and regularly update individualized hospice and palliative care plans.
• Assess and manage pain, symptoms, medications, and treatments in accordance with the patient’s plan of care.
• Serve as a Case Manager when assigned and coordinate care across the patient’s interdisciplinary hospice team.
• Educate patients and families on medications, symptom management, disease progression, and comfort-focused care.
• Communicate changes in patient condition and care needs with physicians and members of the interdisciplinary team.
• Maintain accurate, timely clinical documentation and ensure continuity of care.
• Support patients and families with their physical, emotional, and end-of-life care needs.
• Coordinate necessary equipment, supplies, and services to support quality patient care.
• Participate in interdisciplinary team meetings, quality improvement activities, and continuing education.

What You Will Do

Required Qualifications

• Is a graduate of an accredited school of professional nursing, BSN preferred.


• Currently licensed to practice as a RN in the state of practice.


• If IV or ET certified, is currently certified and recognized by the state(s) where currently practicing as an RN.


• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.


• Possess and maintains a valid driver’s license.

Pay Ranges: $53-$57 commensurate with experience
