Job summary

Lorian Health seeks a Home Health RN in Long Beach, CA. You will provide skilled nursing care at patients' homes, conduct regular assessments, administer treatments, and coordinate with physicians and families to ensure seamless care.

A BSN is preferred, with an active RN license and at least 1 year in home health or hospice. You will work on a pay-per-visit model with a comprehensive benefits package and opportunities for professional growth within a collaborative team committed to

Qualifications

• RN license in the state of practice.
• Minimum 1 year of experience in Home Health or Hospice.
• IV/ET certification if applicable and current in state(s) practicing as RN.
• Valid driver’s license.

Responsibilities

• Provide skilled nursing care per the nursing process and Plan of Care.
• Assess and monitor patient condition; re-evaluate needs regularly.
• Administer medications and treatments within scope and competency.
• Communicate changes in patient condition to physicians and families to ensure continuity of care.
• Educate patients and families on medications, disease management, safety and self-care.
• Complete accurate clinical documentation per policy and regulatory requirements.
• Promote safe, efficient care environment and supervise staff as needed.

Location: Long Beach Southwest

Schedule: Monday - Friday with on call weekends rotation

As a Home Health RN, you will:

• Provide skilled, compassionate nursing care in accordance with the nursing process, physician orders, and the patient’s individualized Plan of Care (POC).
• Assess and monitor patient condition, regularly re-evaluating needs.
• Administer medications, treatments, and specialized procedures within scope of practice, training, and demonstrated competency.
• Communicate changes in patient condition and care needs with physicians, supervisors, patients, and families/caregivers to promote continuity of care.
• Educate patients and families/caregivers on medications, treatment plans, disease management, safety, and self-care to support independence.
• Complete accurate, timely, and thorough clinical documentation in accordance with Lorian policies, procedures, and regulatory requirements.
• Promote a safe, efficient care environment while supporting activities of daily living, appropriately delegating/supervising staff, and maintaining professional knowledge through ongoing education.

Required Qualifications

• Is a graduate of an accredited school of professional nursing, BSN preferred.
• Currently licensed to practice as a RN in the state of practice.
• If IV or ET certified, is currently certified and recognized by the state(s) where currently practicing as an RN.
• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
• Possess and maintains a valid driver’s license.

This position is based on a pay per visit compensation model. Pay per visit ranges from $100 to $125 visit commensurate with experience and depending on type of visit.
