Lorian Health in San Diego, CA, seeks an experienced LPN/LVN to provide in-home nursing care under RN supervision. You will support patients with daily care, monitor conditions, and help ensure safe, high-quality home health services.

Requirements include a current LPN/LVN license in California, graduation from an accredited program, a year of home health or hospice experience, CPR/BLS certification, and a valid driver’s license with reliable insured vehicle.

Qualifications

• Current LPN/LVN license in CA.
• Graduate of an accredited practical/vocational nursing program.
• Minimum 1 year of recent Home Health or Hospice experience.
• Current CPR/BLS certification.

Responsabilités

• Provide nursing care according to the RN-developed Plan of Care.
• Monitor and document changes in patient condition; report to RN/physician as needed.
• Assist with activities of daily living to promote patient independence.
• Educate patients and caregivers within LPN/LVN scope of practice.
• Maintain accurate EMR documentation.
• Participate in case conferences and coordinate care with the home health team.
• Follow infection-control, HIPAA, and company policies.

Job Summary

We’re looking for a skilled and compassionate LPN/LVN to provide in‑home nursing care under the supervision of a Registered Nurse (RN). You’ll support patients with daily care needs, monitor their condition, and help ensure safe, high‑quality home health services.

Responsibilities

• Provide nursing care according to the RN‑developed Plan of Care.
• Monitor and document changes in patient condition; report to RN/physician as needed.
• Assist with activities of daily living to promote patient independence.
• Educate patients and caregivers within LPN/LVN scope of practice.
• Maintain accurate EMR documentation.
• Participate in case conferences and coordinate care with the home health team.
• Follow infection‑control, HIPAA, and company policies.

Requirements

• Current LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
• Current CPR/BLS certification.
• Valid driver’s license, reliable insured vehicle.
• Strong communication, clinical judgment, and ability to work independently.

Physical Requirements

• Ability to lift/transfer patients and perform physical caregiving tasks.
• Ability to pass required physical exams and annual PPD testing.

Hourly range from $34 to $42 commensurate with experience.
