Job summary

Ascendconciergehomecare is looking for a Registered Nurse in North Wales, PA, responsible for assisting with on-call case management and visits, ensuring patient care needs are met. Ideal candidates will have an Associate's Degree in Nursing and a State Certified RN license.

Responsibilities

• Completes initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluates the patients' nursing needs.
• Uses assessment data to determine care plan needs.
• Collaborates with healthcare team to provide continuous care.
• Counsels patient and family in meeting end of life needs.
• Initiates appropriate nursing procedures.
• Communicates with the physician regarding patient's needs.
• Supervises the Hospice Aides.

Hours: PRN Weekends

Responsibilities

The Registered Nurse will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

1. Completing an initial assessment of patient and family to determine hospice care needs
2. Regularly re-evaluates the patients nursing needs
3. Use assessment data to determine care plan needs
4. Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated
5. Counsels the patient and family in meeting end of life needs
6. Initiates appropriate preventative and palliative nursing procedures
7. Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
8. Supervises the Hospice Aides

Education

Associate's Degree in Nursing

Professional Experience

• Must have a State Certified RN license
• 2 years' experience as a professional nurse, hospice experience preferred
• Must have a valid Driver's license and reliable transportation
• Must be willing to travel
