Ascend Hospice & Palliative Care PA is seeking a Registered Nurse for on-site weekend PRN coverage in North Wales, PA. The RN will support on-call, case management and patient visits to ensure continuous, well-coordinated hospice care.

Responsibilities include assessments, care planning, team collaboration, and family counseling. Requires an RN license, 2 years nursing experience (hospice preferred), valid driver's license, willingness to travel, and strong communication skills.

Qualifications

• State RN license required.
• 2 years of professional nursing experience; hospice experience preferred.
• Valid driver's license and reliable transportation.
• Willing to travel.
• Strong verbal and written communication skills.

Responsabilités

• Complete an initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluate the patient's nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the entire healthcare team, patient and family, to direct the work of the team for continuous, well-coordinated care.
• Counsel the patient and family in meeting end-of-life needs.
• Initiate appropriate preventative and palliative nursing procedures.
• Communicate with the physician regarding patient needs and changes in condition.
• Supervise the Hospice Aides.

Hours: PRN Weekends

The Registered Nurse will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

Education:

Associate's Degree in Nursing

Professional Experience:

• Must have a State Certified RN license
• 2 years' experience as a professional nurse, hospice experience preferred
• Must have a valid Driver's license and reliable transportation
• Must be willing to travel
• Strong verbal and written communication skills.
