Job summary

Ascendconciergehomecare is looking for a Registered Nurse in North Wales, PA, responsible for assisting with on-call case management and visits, ensuring patient care needs are met. Ideal candidates will have an Associate's Degree in Nursing and a State Certified RN license.

Qualifications

Must have a State Certified RN license.
2 years' experience as a professional nurse, hospice experience preferred.
Must have a valid Driver's license and reliable transportation.
Must be willing to travel.

Responsibilities

Completes initial assessment of patient and family to determine hospice care needs.
Regularly re-evaluates the patients' nursing needs.
Uses assessment data to determine care plan needs.
Collaborates with healthcare team to provide continuous care.
Counsels patient and family in meeting end of life needs.
Initiates appropriate nursing procedures.
Communicates with the physician regarding patient's needs.
Supervises the Hospice Aides.
