Zusammenfassung

Capital Hospice & Palliative Care is seeking a full-time Hospice RN to organize and direct hospice care with emphasis on community health education. You will assess, plan, and implement patient care while collaborating with families and the interdisciplinary team.

You will complete comprehensive assessments, develop care plans with IDG/PCP, educate patients on hospice philosophy, and ensure accurate clinical documentation.

Qualifikationen

• Graduate of an accredited school of nursing.
• Valid RN license in Illinois.
• Valid IL driver's license and auto insurance.
• Hospice experience preferred but not required.

Aufgaben

• Complete initial comprehensive physical assessment and history.
• Ongoing patient and family assessments to determine hospice plan.
• Develop care plans with IDG, PCP, patient and families.
• Educate patients and families on hospice services and philosophy.
• Supervise plan of care as per regulations.
• Maintain and submit accurate clinical notes.
• Attend interdisciplinary group meetings.
