Job summary

Northern Arizona Hospice is seeking a dedicated Registered Nurse Case Manager to deliver individualized hospice care at patients’ homes. You will develop and implement plans of care in collaboration with the care team, focusing on physical, psychosocial, emotional, and spiritual well-being.

You will evaluate and adjust plans, ensure timely documentation, and support families through difficult times. A valid RN license and reliable transportation are required for success in this role.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Valid driver’s license and access to a dependable vehicle, or reliable transportation.

Responsibilities

• Initial nursing evaluation within 48 hours of referral.
• Assess patient/family physical, psychosocial, and environmental needs and re-assess every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family response to care.
• Collaborate with patient/family, attending physician and IDG daily.
• Instruct and supervise patient/family in self-care when appropriate.
• Maintain accurate clinical notes regarding patient condition.
• Provide care in diverse home settings regardless of age, race, creed, or disability.
