Job summary

HopeWest is seeking a Hospice Registered Nurse for Grand Junction, CO, delivering human-centered care across home settings. You will join an interdisciplinary team, manage a patient caseload, educate families, and support goal-concordant care with compassion.

The role is Mon-Fri, with a $10,000 sign-on bonus and competitive pay for Colorado-licensed RNs with CPR. Hospice or home health experience is preferred, emphasizing strong patient-family relationships.

Qualifications

• Minimum 1–2 years of nursing experience preferred.
• Experience in home health, hospice, or acute care strongly preferred.
• Current Colorado RN license maintained throughout employment.
• Current CPR certification maintained throughout employment.
• Valid Colorado driver’s license, reliable transportation, and proof of insurance.
• Strong communication and interpersonal skills.
• Ability to navigate emotional or high-stress situations with professionalism and compassion.
• Experience with EHR systems and Microsoft Office applications.

Responsibilities

• Provide relationship-based care and lead the interdisciplinary team.
• Manage an assigned patient caseload with a focus on symptom management and comfort.
• Educate patients and caregivers to identify goals of care and enhance confidence in home care.
• Deliver care wherever patients call home (private homes, nursing homes, group homes or assisted living).

Primary RN – Grand Junction Community Hospice | Monday–Friday 8am-5pm| $10,000 Sign-On Bonus

Schedule: Monday–Friday, 8:00am–5:00pm, with occasional on-call/weekend requirement
Location: Grand Junction, Colorado
Sign-On Bonus: $10,000
Pay Range:
ASN: $38.02–$41.75/hour
BSN: $39.20–$43.05/hour
