Job summary

Oakbrook Hospice and Palliative in Oakbrook, IL is seeking a Registered Nurse to perform comprehensive patient assessments (including OASIS), develop and evaluate plans of care, and coordinate with the health team to ensure safe, compliant care.

Qualifications

• Proficient in completing initial and ongoing comprehensive patient assessments.

Responsibilities

• Complete initial and ongoing comprehensive patient assessments, including OASIS as applicable.
• Develop, implement and evaluate the patient's plan of care per physician orders.
• Document OASIS data and follow up with physicians on medication discrepancies.
• Provide and document patient and family education on pain management, medications, and care plans.
• Coordinate with home health team and ensure smooth transfer and discharge processes.
• Monitor patient status, notify physician of changes, and assist with care coordination.
