Qualifications

At least 1 year of acute care experience in an institutional setting.
1 year of home care experience preferred.
Current licensure in the state.
Understanding of hospice philosophy and needs of terminally ill patients.
Flexible in work hours; willingness to travel within Bristol Hospice Service Area.
Ability to maintain comprehensive working knowledge of information systems and software; proficient in charting.

Responsibilities

Perform initial and ongoing patient/family assessments to determine hospice needs.
Provide comprehensive physical assessment and history.
Deliver professional nursing care using the nursing process.
Develop, implement, and revise care plans with regular re-evaluation.
Initiate and adjust care plans as patient status changes.
Collaborate with physician and inter-disciplinary team; communicate patient needs.
