Job summary

Bristol Hospice-CALIFORNIA LLC in Merced, CA is seeking a licensed LVN/LPN to join our team on a PRN, hourly basis. You will provide direct patient care and end-of-life support in a compassionate, family-centered hospice setting.

The role requires current LVN/LPN licensure in California and about three years of nursing experience; hospice or medical-surgical experience preferred. Strong communication, flexibility, and willingness to work varied shifts are essential.

Qualifications

• Must be licensed as LPN/LVN in California.
• Three years nursing experience; hospice or medical/surgical preferred.
• Must understand hospice philosophy and care needs of the terminally ill.
• Flexible in work hours.
• Willing to maintain knowledge of information systems and software.

Responsibilities

• Provide direct patient care per State Nurse Practice Act, including end-of-life care.
• Implement current nursing care following assessment and plan of care.
• Document patient services accurately and timely to reflect the plan of care.
• Educate patient and family/caregiver on diagnosis and self-care.
• Coordinate hospice services with the interdisciplinary team.
• Use and prepare equipment with aseptic technique.
• Provide pain and symptom management.
• Evaluate patient response to treatments.
• Participate in professional growth and QA activities.
• Perform other duties as assigned by the registered nurse.

Education

Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) - California license
