Job summary

Bristol Hospice in Merced, CA is seeking a compassionate part-time Registered Nurse to cover after-hours and weekend visits. You will manage admissions, triage calls, and coordinate with the care team to ensure timely patient support in the field.

Ideal candidates are flexible, have hospice knowledge, and can lift up to 50 lbs. This role involves travel within the Bristol Hospice service area and uses standard software systems for documentation.

Qualifications

• Must be a Registered Nurse with knowledge of hospice; telephone triage preferred
• Must have current licensure in good standing with the state
• Must understand the philosophy of hospice and the needs of the terminally ill
• Must be flexible in work hours
• Must show aptitude of computer data entry and use of current software systems
• Ability to travel throughout the Bristol Hospice Service Area and lift up to 50 lbs.

Responsibilities

• Will perform after-hours and weekend visits and admissions as needed.
• Follow up on phone calls by directing the call to the DPCS, RN Clinical Supervisor/Manager or RN Case Manager and/or take care of the need per scope of practice
• Dispatch field nurses and other staff to patient residences in a timely manner
• Refill prescriptions, order supplies and DME, and assist in coordination for patients and families
• Document calls, conversations and outcomes in the call-in software system
• Receive start-of-shift reports and give end-of-shift reports per location workflow
• All other duties as assigned
