St. Croix Hospice is seeking a Registered Nurse to provide day-to-day hospice nursing care under the guidance of the RN Case Manager. The role emphasizes comfort, quality of life, and dignity for patients in home settings.

The RN will administer medications, document care, and participate in Plan of Care development with an interdisciplinary team, while ensuring HIPAA compliance and patient privacy. Travel to patient homes is required as part of a home health hospice program.

Qualifications

• Graduate of an accredited school of nursing.
• Associate degree in nursing required.
• Must hold a current RN license in state(s) of practice.
• Multistate license acceptable where permitted by state rules.
• Ability to pass background and drug screening.

Responsibilities

• Provide day-to-day hospice nursing care based on the Plan of Care.
• Administer medications and treatments as prescribed.
• Maintain accurate patient records and documentation.
• Develop and revise Plan of Care with the care team.
• Collaborate with family/caregivers and the interdisciplinary team.
• Ensure HIPAA compliance and patient confidentiality.
• Assist with activities of daily living as needed.
