St. Croix Hospice is seeking a compassionate Registered Nurse Case Manager to deliver holistic, in-home hospice care. You will work with a care team to create personalized plans and guide families through end-of-life conversations with clinical excellence.

The role requires an accredited nursing degree and 1+ year acute care experience, with hospice/home health preferred. A driver’s license and reliable transportation are necessary to support in-home visits.

Qualificações

• RN with an accredited nursing degree (ADN/BSN/MSN) required.
• 1+ year of recent acute care experience; hospice/home health preferred.
• Ability to complete initial and ongoing comprehensive assessments to determine hospice needs.

Responsabilidades

• Develop individualized care plans with patient and family, updating as needs evolve.
• Lead end-of-life conversations and ensure symptom management with empathy and clinical excellence.
• Administer medications and treatments as prescribed and educate patients and caregivers.
• Rotate on-call duties and ensure seamless care transitions.
